Maximum Award Amount: $1,000,000

Total Number of Anticipated Recipients: 10

Solicitation Priorities:

  • Promoting community re-integration and health of individuals returning to the community after incarceration, hospitalization, or institutionalization
  • Preventing accidental injury, violence, or suicide and suicide thoughts and behavior
  • Increasing affordable healthy food option and opportunities for physical activity
  • Changing community norms and reducing structural barriers related to health promoting behaviors
  • Improving community attitudes towards sociodemographic groups or towards individuals with certain health conditions
  • Enhancing the ability of community-dwelling adults to age in place or individuals with disabilities to live independently
